Attending Another Institution-Student Information

In some cases, the donor and the GCSC Foundation will approve for a scholarship award to be sent to another educational institution.  Since the GCSC Foundation does not have the recipient’s student information on file, we must have the information sent to us by the scholarship recipient. 

COMPLETE THIS FORM: In order for your scholarship award to be sent to your institution, please complete and submit this form to the Foundation: https://www.gulfcoast.edu/foundation-forms/institutional-information.html.

You will be asked to enter the following information. Please have this available before you start the form: 

  1. The name of your institution
  2. Your school issued email address
  3. Your school issued Student ID Number (VERY IMPORTANT)
  4. The school’s address where to send your scholarship award (Please verify with the school; this is usually the Bursar’s Office, Student Services, or Financial Aid)

Failure to follow the instructions or submit an incomplete form may cause your scholarship award payment to be delayed!
